Telehealth Informed Consent

This Telehealth Informed Consent ("Consent") describes how telehealth services are provided through clearpathcompounds.com, a site operated by MHMNUTRA LLC, a Delaware limited liability company ("Clearpath Compounding", "we", "us", "our"). By using our telehealth services, you acknowledge that you have read, understood, and agree to this Consent.
1. What Telehealth Is
Telehealth is the delivery of healthcare services through the use of electronic communications — including secure web forms, asynchronous messaging, video, and audio — between a patient and a licensed healthcare provider located in a different physical location.
2. Services You May Receive
- Review of your health intake information by a licensed healthcare provider
- Asynchronous (store-and-forward) medical evaluation based on your written intake
- If clinically appropriate, the issuance of a prescription to be dispensed by a licensed U.S. compounding pharmacy
- Follow-up communications regarding your treatment, including refill review
3. Provider Independence
All clinical decisions — including whether to issue a prescription, the choice of medication, the dose, and any monitoring requirements — are made solely and independently by the licensed healthcare provider reviewing your case, based on their professional medical judgment. Clearpath Compounding does not direct, influence, or override clinical decisions.
4. Risks & Limitations of Telehealth
- Limited physical examination. A telehealth provider cannot perform an in-person physical exam. Your intake answers and photos (if requested) are the basis of the evaluation.
- Possibility of misdiagnosis. As with any medical service, there is a possibility that information provided will be incomplete, inaccurate, or insufficient to support a diagnosis.
- Technology disruptions. Electronic transmissions may be delayed, interrupted, or fail. We use industry-standard security but cannot guarantee that systems will be free of all risks.
- Not suitable for emergencies. Telehealth is not appropriate for urgent or emergency conditions. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
5. Patient Responsibilities
You agree to:
- Provide truthful, accurate, and complete information about your medical history, current symptoms, medications, and allergies
- Confirm your identity and U.S. residency, and accurately report your physical location at the time of the consultation
- Disclose any known pregnancy, breastfeeding, or planned pregnancy
- Follow all instructions from your prescribing provider and the dispensing pharmacist
- Maintain regular in-person care with a primary care provider; telehealth does not replace ongoing in-person care
- Report any adverse reactions, side effects, or changes in your condition promptly
6. Prescription Process & No Guarantee
Completion of an intake form, payment of a consultation fee, or any other interaction with the platform does not guarantee that a prescription will be issued. A licensed provider will only issue a prescription when, in their independent clinical judgment, it is medically appropriate for the patient. If a prescription is declined, you will be notified and any applicable fees will be handled per our Refund Policy.
7. Off-Label & Compounded Medications
Some medications dispensed through this Site may be compounded (custom-prepared at a licensed pharmacy) or prescribed off-label (used for a purpose other than the FDA-approved indication). Off-label and compounded prescribing is a legal and common medical practice, but it is important you understand:
- Compounded medications are not FDA-approved drug products in the same way commercially-manufactured medications are
- Your provider will discuss the rationale, evidence, and risks of any off-label or compounded prescription as part of their evaluation
- You should ask any questions you have before starting treatment
8. Privacy & Security of Health Information
Information you provide during a telehealth interaction — including intake answers, photos, and provider communications — constitutes protected health information ("PHI"). PHI is handled in accordance with applicable U.S. health-privacy laws, including HIPAA where applicable. See our HIPAA Notice of Privacy Practices and our Privacy Policy for full details.
9. Communications
By providing your email address and/or mobile number, you consent to receive communications related to your healthcare and orders, including:
- Provider messages and intake follow-ups
- Prescription and shipping notifications
- Refill reminders and care updates
Standard messaging rates may apply for SMS. You may opt out of non-clinical messaging at any time.
10. Right to Refuse or Withdraw Consent
Your participation in telehealth is voluntary. You may withdraw this consent at any time by contacting us at help@clearpathcompounds.com. Withdrawal does not affect the lawfulness of services already delivered.
11. Geographic Restrictions
Telehealth services are available only to individuals physically located in, and residents of, U.S. states where our affiliated licensed providers are authorized to practice and where dispensing of compounded prescription medication is permitted. We do not serve patients located outside the United States.
